In consideration of receiving a tattoo from See See Kwan as a tattoo art practitioner, I agree to the following (please check the boxes):*

I have been fully informed of the inherent risks associated with getting a tattoo.I have been fully informed of the inherent risks associated with getting a tattoo.

I understand that these risks, known and unknown, can lead to injury including but not limited to: infection, scarring, difficulties in detection of melanoma, allergic reactions to tattoo pigment, soap, and/or Secondskin bandage.

I waive and release to the fullest extent permitted by law See See Kwan from all liability whatsoever, including but not limited to, any or all claims that I may have for personal injury or dissatisfaction which may arise from the tattoo application.

I have been given instructions on the aftercare of my tattoo on See See Kwan's website. I acknowledge that it is possible that the tattoo can become infected if instructions are not carefully followed.

I release the right to any photographs taken of me and the tattoo and give consent in advance for their reproduction in electronic form. If you do not check the box, please inform See See Kwan that you do not want pictures taken.

I have been given full opportunity to ask any questions about the procedure and application of my tattoo and all my questions, if any, have been answered to my total satisfaction.

I am not under the influence of alcohol or drugs and have truthfully represented to See See Kwan that obtaining a tattoo is by my choice alone. I consent to any procedures required to receive a tattoo by See See Kwan.

I am of legal age to receive a tattoo and have provided valid proof of age and identification. I have read the agreement, I understand it, and I agree to be bound by it.
